Cost bands allow rates to vary with age, salary, tobacco use, and / or other values.

All rates in iSolved are determined by monthly values and then broken up by pay schedule to determine per-pay deductions for running payroll.

By Age

1. When is age measured? ex. As of Beginning of Plan Year; As of Jan 1st; As of 1st of Month After Birthday
   1. Participant date of birth must be entered in iSolved to properly calculate the premium
2. Whose age determines the cost? ex. Dependent cost is based on Employee Age; Dependent cost is based on Dependent Age
3. Is the cost of coverage capped at a number of dependents?
   1. How  many dependents? ex. First 3 Children
   2. Under what age? ex. Under 21
   3. What sort order? ex. Descending Age

By Salary

1. What earnings count towards salary? ex. Base Salary; Base Salary + Commissions + Bonuses
2. What is the salary frequency? ex. Annual, Monthly, Weekly
3. When is salary measured? ex. Current; At Beginning of Plan Year; At Beginning of Calendar Year; At Last Day of Last Month

By Tobacco Use

1. Whose tobacco use determines the cost? ex. Dependent cost is based on Employee Use; Dependent cost is based on Dependent Use
